How to Interpret a Bill and Resolve Billing Inquiries:
Your statement will include a unique account number, as well as the date of service, procedure that was performed, amount charged, and the name of the group or individual providing the services.
By contacting the above number you will be able to provide new or updated insurance information and authorization to allow the insurance company to pay directly. When calling, please communicate to the customer service representative the account number provided on your statement. If you have received an Explanation of Benefits (EOB) from your insurance company, please have this available to provide additional information.
Any payments or adjustments that have been made will be listed below the original charge. It is your responsibility to ensure settlement of any insurance claim. You will continue to receive a bill until the insurance company has covered the balance due in full or you have fulfilled your obligation of the payment.
